Heath Screening Form
Before you start any of our physically active course, we will ask you to complete a 'Health Screening' form. This will give you the opportunity to raise any issues or concerns about your current condition with your tutor, who will ensure that you get the most from your course and be able to monitor your progress. Should you have any serious concerns about your health in relation to your ability to engage in physical activity or have not been physically active for some time, please seek the advice of your doctor before enrolling. Please note that we are not able to offer medical advice.
